REGULATIONS AND GENERAL TERMS OF SERVICE

§1 General Provisions

1. These Regulations define the rules for the provision of services by SHAPE CARE.

2. Booking an appointment constitutes acceptance of these Regulations and General Terms of Service.

3. All services provided at SHAPE CARE are strictly medical in nature and performed by qualified professionals.

 

§2 Scope of Services

1. SHAPE CARE provides physiotherapy, manual therapy, and medical massage services.

2. The scope of therapy is determined individually based on medical history and functional assessment.

3. The therapist reserves the right to refuse treatment if medical contraindications are identified.

 

§3 Appointments and Cancellations (Cancellation Policy)

1. Services are provided by prior appointment only.

2. Appointments must be cancelled or rescheduled at least 48 hours before the scheduled time.

3. Failure to cancel within the required timeframe or failure to attend an appointment may result in the patient being charged the full cost of the scheduled session.

4. In case of late arrival, the session duration may be shortened without reduction of the fee.

 

§4 Payments and Invoicing (Invoice Policy)

1. Payment is due immediately after the session unless otherwise agreed.

2. SHAPE CARE issues invoices in accordance with Belgian law.

3. In case of late payment, statutory interest and administrative fees may be applied.

4. The patient is responsible for providing accurate billing information.

 

§5 Patient Responsibilities

1. The patient must provide accurate and complete information regarding their health condition.

2. The patient agrees to follow the therapist’s professional recommendations.

3. The patient must immediately inform the therapist if any discomfort or adverse reaction occurs during treatment.

 

§6 Liability

1. Services are provided with due professional care and in accordance with current medical standards.

2. SHAPE CARE is not liable for consequences resulting from incomplete or inaccurate health information provided by the patient.

3. No specific therapeutic outcome can be guaranteed, as results depend on individual health conditions and biological response.

 

§7 Data Protection (Privacy Policy)

1. Personal data is processed in compliance with the EU General Data Protection Regulation (GDPR).

2. Data is collected and processed solely for the purpose of providing medical services, maintaining medical records, and handling billing matters.

3. Detailed information regarding data processing is available in the Privacy Policy at www.shapecaretherapy.com.

 

§8 Final Provisions

1. These Regulations enter into force upon publication on the SHAPE CARE website.

2. SHAPE CARE reserves the right to amend these Regulations at any time.

3. Matters not covered by these Regulations shall be governed by applicable Belgian law.
